work car 00 suzuki swift/geo metro

I have a 2000 model suzuki swift ( geo metro) it has a 1.3L with 5 speed trans. my problem is, all the gears shift and pull fine. its not slipping in any gear. first through fourth i can rev up to whatever and nothing will happen. in fifth, ill get to about 45 or so and im really on it and it will slip out of fifth. it happens between 45 and up. but i can hold it in fifth and it pulls with no problem. also if i kinda take it easy and accelerate slowly up to speed it does fine. and finally after it acts up like this it will straighten up and i wont have a singel problem. i can get on it hard in fifth and it wont slip out. any help would be great guys, i drive 182 miles a day and i dont want to be stranded. im also going to pick up a haynes service manual tomorrow. but if you have any idea, PLEASE let me know THANKS!!!!!!!!!! oh it has 49500 miles on this baby
